have found that many simple problems have become major headaches because of a
lack of documentation. However as it seems most practice and business management
courses have little or no IT components, most managers are aware of what they
should have as documentation Listed below are sample/pro forma documents that
you may download and modify for your business or practice. We recommend that you
create and store two copies in four-ring A4 binders. One copy stays on-site for
the staff to access and another copy goes off-site to aid disaster recovery in
case of fire or theft.

there is a common theme throughout all the documentation. All documentation
have:
